Fire Department Feedback Form

As responders, you are an important part of the quality improvement process. This form provides a way to notify EHP/HVA leadership of incidents that may warrant review and to recognize exceptional performance by dispatch personnel or field crews.

Please use this form to report concerns, observations, commendations, or other feedback related to dispatch operations or patient care. Examples include, but are not limited to:

Quality Improvement Concerns

• Concerns regarding the level of patient care provided
• Scene safety information that was not communicated or was communicated inaccurately
• Questions regarding the Emergency Medical Dispatch (EMD) protocol selected
• Concerns about the final call coding, determinant, or response priority
• Communication or coordination issues affecting patient care or responder safety

Recognition and Commendations

• A dispatcher successfully guiding a caller through CPR, childbirth, choking intervention, bleeding control, or other pre-arrival instructions
• A call taker or dispatcher who demonstrated exceptional professionalism, compassion, or problem-solving
• A paramedic, EMT, or crew member who provided outstanding patient care or service
• Any situation where a dispatcher, call taker, or field provider went above and beyond expectations

All submissions will be reviewed by EHP/HVA leadership as part of our ongoing commitment to excellence, accountability, and continuous improvement.
